Abstract: Abstract of thesis entitled Clinical Study on Acupoints Application on San Fu Days for Treating Bronchial Asthma Submitted by ZHANG Wei for the degree of Master of Philosophy at The University of Hong Kong in December 2012 The research project comprised two interrelated studies, the first was meta-analysis with an aim to evaluate the literature regarding therapeutic effects of acupoints application on San Fu Days for adult asthma patients; the second was a prospective, randomized, double blinded and placebo controlled trial (RCT) aiming to evaluate the efficacy of acupoints application on San Fu Days in preventing and reducing the frequency and severity of asthmatic attacks. In meta-analysis study, eleven electronic databases were retrieved. RCTs for adult asthma patients were included. Statistical analysis was on the basis of the requirements of Cochrane library. Of 1048 articles, five RCTs had 534 mild patients with Treat Group (TG, n=282) and Control Group (CG, n=252) included. All the studies had lower quality with the Jadad score range 1 to 2. There was no 2 2 statistical heterogeneity among these studies (X =6.84; p=0.14; I =42%), and also no obvious publication bias (p>-t-=0.785, 95% CI: -7.2527, 8.7553). CG had significant better valid response rate than TG (OR=3.97, z=7.75, pIn the RCT, patients whom above 13 years old and fulfilled the asthma diagnosis were recruited. Participants received the treatments onto acupoints by patches of herbal drugs in TG and placebo in CG on 19 July, 29 July and 8 August 2010 respectively. Follow ups were scheduled to evaluate the sustained therapeutic rd th th th efficacy on 3, 6, 9 and 12 month after treatment. 323 of 451 eligible patients were successfully randomized into TG (n=165) and CG (n=158). Ultimately, only 42.41% (114/323) of them including 60 patients in TG and 54 patients in CG completed this study. After treatment, the value of PIF (L/s) was obviously increased with 4.35 (3.30, 6.37) in TG vs 4.02 (2.96, 5.37) in CG (Z=-2.066, p=0.039). The improvement rate value of PEF (L/s) in moderate persistent patients in TG was distinctly increased up to 16.19% (Z= -2.083, p=0.037). The FEV /FVC (%) was significantly increased by 4.52% (-0.43, 10.78) in TG vs -0.85% (-5.95, 1.49) in CG in moderate persistent patients (Z=-2.789, p=0.005). There were no significant changes in FVC, FEV, FEV /predicted, heart rate (HR) 1 1 and oxygen saturation (SaO ) (all p>0.05). All the clinic symptoms in TG and CG were not found to be significant different in the whole study (p>0.05). During the follow-ups, there was no difference in FEV /FVC and FEV / predicted between 1 1 TG and CG (p=0.249 and p=0.687 respectively). The present study found this therapy was safe by controlled treatment time. It was lack statistical evidence to prove it could prevent and reduce the frequency and severity of asthmatic attacks.
